NOTE TO PARENTS:
Our travel kits are put together by staff based on the information you provide about your child's age, interests and destination. We must have one week's notice because of the preparation time involved. This service is limited to Lisle Library patrons in good standing. Kits will be checked out to the person indicated on this form and you will be called or emailed when the items are ready. |
